How do I use FastScreen to check my vaccination card?

Please follow these simple steps to scan your vaccination card.

1. Send a text message to the FastScreen site at 415-429-5567 with the code provided to you by your organization

2. After receiving a reply text, click the link in your text message to access the secure site

3. Confirm your contact information

   1. Confirm your contact information
   2. Check box for “Myself”
4. Choose “Vaccination” from the list if asked
5. Select an image of your vaccination card to scan

6. Receive confirmation of your confirmed vaccination card scan
