How to Download and Submit Digital or Downloadable Forms

Your school may require to fill out or upload certain forms for them to keep on file.

Curacubby offers two different types of forms:

digital forms: which can be completed from within your Curacubby portal
downloadable files: which require you to download the form to complete, and then re-upload the completed form. Here are steps on how to complete both types of forms.

Digital Forms:

Digital forms can be completed, signed, and submitted through Curacubby.

1. Log in to your Curacubby account and click “Forms and Files.”  
2. Click "Forms for [your child]."
3. Complete the necessary fields. 
4. Click "Submit!" to submit your form.

*Note: If you are not ready to submit your form yet, you can select "Save and complete later" and this will save all of the information you have entered to be submitted at a later time.

Downloadable Files:

Downloadable forms must be downloaded, completed, printed, and signed.  

1. Log in to your Curacubby account and click “Forms and Files.”  
2. Click "Forms for [your child]" and scroll down to "Downloadable Files."
3. Click on the Form Title to download the file. 
   IMPORTANT: Open the file in Adobe Acrobat or Preview.  Do not edit the form directly in your browser, as it will not save your information.  
4. After you have completed the form, save the file.   
5. Once the form is completed, and the file is saved, click “Upload File.”  
6. Type a description, such as the Title of the form, then click “Choose File.” Choose the completed file from your computer.
7. Click “Upload” and you’re done.
